Inquest Reveals Breaches Leading to Jordan Keil’s Tragic Death

The inquest into the death of Jordan Keil has uncovered significant failures in mental health care, culminating in the tragic events that led to his escape from a moving vehicle. A nurse from the Tiaho Mai mental health unit described the harrowing moment when Jordan Ihaia Keanu Keil leapt from the back seat of a car on the Southern Motorway. The vehicle was transporting him from Auckland City Hospital to the secure mental health facility when he exited the moving car. The nurse recounted how she was forced to abruptly halt the vehicle, underscoring the chaotic nature of the situation.
The inquest also revealed details surrounding Keil’s earlier escape from the Middlemore Hospital mental health unit. His mother, Debbie Thorpe, discovered his body in their garden 16 hours after his escape, just five meters from the broken window he used to flee. The circumstances leading to Keil’s death in February 2022 were examined in the Auckland District Court, with the proceedings attended by grieving family and friends who sought answers.
Coroner Rachael Schmidt-McCleave presided over the inquest, which focused on the series of lapses in protocol that may have contributed to the tragic outcome. Testimonies from various healthcare professionals illustrated a concerning lack of communication and coordination in Keil’s treatment plan.
